Treatment Agreement: 

Practice Infini works with a treatment agreement in accordance with the Dutch Meidcal Treatment Contracts Act (WGBO). This agreement includes, mong other things, how Practice Infini handles your privacy. The treatment agreement will be sent to you before the intake interview. 

General Data Protection Regulation - GDPR

Practice Infini maintains a register of processing activities. This register inlcudes: 

A. a description of the categories of personal data (client data) that Practice infini processes; 

B. a description on the purposes of wich Practice Infini Processes  personal data; 

C. what rights data subjects (clients) have and how they can exercise those rights; 

D. what organizational and technica; measures Practice Infini takes to secure the personal data; 

E. how Practice Infini retains the personal data; 

F. how Practice Infini handles a data breach
